Here’s what Prolonged Exposure therapy looks like step by step…
Step 1: Creating a Safe Space
At the start of our Prolonged Exposure (PE) therapy session, I want you to feel safe and secure. We’ll create a comfortable space where you can unpack and unwind, and I’ll explain the entire process before we begin. We’ll also set up an understanding that you can pause or stop the session at any time if it becomes too overwhelming and continue when you’re ready.
Step 2: Getting Grounded
We’ll begin by taking a few deep breaths to ground ourselves in the present moment. I may ask you to describe a calming place you can visualize in your mind, helping you create a mental safe space.
Step 3: Graded Exposure
The essence of PE is gradually facing the traumatic experiences and memories haunting you. We will start with the less distressing aspects, such as discussing the facts of the event. I may ask questions to help you recall specific details.
Step 4: Revisiting Traumatic Memories
As the session progresses, we will delve deeper into your traumatic memories. I will ask you to recount the event in the present tense, vividly describing your surroundings, emotions, and physical sensations as if you were reliving it. It might feel unsettling initially, but I will guide you through it, reminding you that you are in a safe space.
Step 5: Managing Anxiety
While revisiting these distressing memories, you may experience heightened anxiety. This is entirely normal. I will provide you with coping strategies to manage your anxiety during the session. Breathing techniques and grounding exercises can help you stay in control.
Step 6: Repetition and Habituation
The goal of PE is to reduce your emotional response to the traumatic memories over time. We will repeat this process in subsequent sessions, gradually increasing the level of detail and distress you explore. It’s essential to understand that, through repetition, the traumatic memories will become less distressing, and your anxiety will decrease.
Step 7: Homework Assignments
To make the therapy effective, you will have “homework” assignments between our sessions. This might include listening to your recordings of the traumatic memories or engaging in vivo exposure, where you confront real-life situations that trigger your trauma. I’ll be there to guide you through these assignments.
Step 8: Progress and Healing
As we continue this process, you will notice a reduction in the emotional charge associated with your traumatic memories. The goal is to help you regain control over your life and free yourself from the grip of your past trauma.
